Kaiser Permanente is a large HMO serving California, Colorado, Georgia, Hawaii, Maryland, Virginia, Oregon and Washington (State and D.C.). This particular fixture is located at Kaiser Permanente's French Campus in San Francisco, CA.

This picture was sent in by Steve Stevens. He writes: " The interesting thing [to note] is you have to be almost 6' tall to use this [urinal]. It's next to the elevator on the 6th Ave side [of the building]... As you can see the sink is the same height as the urinal... This building used to [be a part of the] French Hospital before Kaiser took over the property. There are several other restrooms to use [by] those who are shorter ".
